Acid reflux: when does it move from medication to a surgical decision?

Relying on acid suppressants for years isn't a permanent answer. These are the signs that warrant a surgical assessment.

Medication treats the symptom, not the cause

Acid suppressants reduce acid production, so the burning sensation eases. But they do not repair the weak valve between the oesophagus and stomach, nor treat a hiatus hernia if one is present.

This means the reflux itself continues, but its contents are less acidic. For most patients that is entirely sufficient. For a subset, symptoms persist or return the moment medication stops.

Signs that warrant assessment

Symptoms persisting despite regular medication at an adequate dose for an adequate duration.

Symptoms returning immediately on trying to stop medication, meaning permanent dependence rather than treatment.

A hiatus hernia accompanied by clear symptoms.

Oesophageal complications found at endoscopy, such as advanced inflammation or a change in the oesophageal lining requiring close surveillance.

Symptoms outside the digestive tract: chronic night-time cough, recurrent hoarseness, or worsening asthma, where a link to reflux is established.

What precedes any decision

Upper endoscopy to assess the oesophagus and stomach and rule out other causes.

Oesophageal manometry in defined cases, to confirm that oesophageal motility is normal before any procedure that narrows the area.

pH monitoring to document that reflux is genuinely occurring and correlates with symptoms, because some symptoms are attributed to reflux when they are not.

These investigations are not a delay to the decision; they are what make it correct.

What to expect after surgery

Temporary difficulty swallowing is expected in the first weeks because of swelling around the operated area, improving gradually with the dietary progression.

Wind and bloating may increase initially because the ability to belch is reduced after the procedure, and this settles with time.

Many patients reduce or stop medication, but that is decided through follow-up rather than assumed.
