After bariatric surgery: why supplements are not optional

The most common late complication has nothing to do with the surgery itself, and everything to do with stopping supplements.

What changes in absorption

Surgery does not only reduce the amount you eat; it changes the environment in which nutrients are absorbed. Stomach acid decreases, the distance food travels changes in bypass procedures, and the intrinsic factor needed to absorb vitamin B12 is affected.

The result is that a patient can eat a balanced diet and still develop deficiency, because the problem is not intake but absorption.

Why deficiency is not felt early

The body's stores of some nutrients last months or years. So a patient can go a year or two without symptoms despite stopping supplements, and the problem appears only once stores are depleted.

Deficiency symptoms are also deceptive: fatigue, hair loss, poor concentration, tingling in the extremities. These are general symptoms easily attributed to other causes, which is why regular blood tests matter more than how you feel.

What is usually monitored

Your treating team sets the precise list and doses, which differ by procedure. Monitoring typically covers iron and ferritin, vitamin B12, vitamin D and calcium, folate and zinc.

Post-surgical supplement doses differ from what general pharmacy multivitamins provide, which is why you should not swap what your doctor prescribed for a general product without checking.

Protein is a separate issue

Many patients focus on calories and forget protein. Protein deficiency after surgery causes muscle mass to be lost alongside weight, which slows metabolism and affects the final shape of the body.

Your dietitian will set a daily protein target and how to spread it across small meals. This is part of the surgical plan, not an optional extra.

Pregnancy after surgery

Pregnancy raises the body's nutrient requirements at a time when absorption is already altered. This is why pregnancy is usually advised to be postponed for a period after surgery, until weight stabilises and levels are corrected.

If you are planning a pregnancy, tell your team early so the supplement and monitoring plan can be adjusted before conception rather than after.
